You can fill and submit the PAN Change Request Application Form by mentioning the Aadhaar linked PAN in the form. All other PAN/s allotted to you accidentally should be mentioned at item no. 11 of the form and relevant PAN card copy/s should be submitted along with the form for cancellation.
